Unhappy Car Sick Yorkie

Toto will be two years old in March and is still unable to ride in the car. He always gets sick and vomits numerous times. I have had to stop as many as 4 times to clean his crate and put in more papers and pads. The vet told me to give him 1/4 of a dramamine, still he got sick. I have used childrens benadryl and have also tried the pet calming pills. Nothing has worked. I also withhold his food for several hours before traveling. When I even start toward my car, he starts to stiffen up and begins to shake and it's almost impossible to get him in his crate. The only time that I travel with him now is to go the the vet. I have tried taking him on little 5 minute rides, no luck! Toto is sort of a timid yorkie. Loud noises, sudden movements or anything out of the ordinary seem to scare him to death. He also does not like to be picked up. I have worked and worked with him but nothing seems to help. He has never been mistreated in any way. Has anyone else had any of these problems?
