When the TSH is low and the T4 and T3 are also low, it most often means central hypothyroidism. That is where the thyroid hormones are low because the pituitary is not producing enough TSH (which is thyroid stimulating hormone) or the hypothalmus is not producing enough of the hormone it sends to the pituitary to stimulate it. So, it is really a pituitary or hypothalmic problem, rather than a thyroid problem. Still, central hypothyroidism is treated with more thyroid hormone.
