Cash does this also. I try to watch him when i know he needs to poop and what he does is go to his litter box squats and starts to poop but before the poop comes out he'll hop out of there. I think he's scared he's going to step in it. This happens everytime and when he hops out, that's when the poop lands on the floor and not the pad. So i try to keep a eye on him and when he has to go, i make sure he stays in there and sometimes I hold him down in there cuz he has balance problems (he lifts his hind legs up too high). This usually works but only when I'm watching. when i'm not, it's the chances are 50-50 that i find poop on the floor.
