In that case, I would have restricted access to the bathroom rather than put a pee-pad down, but that's just me. I think you are confusing her.
Loki is completely crate and housetrained, HOWEVER, he likes/needs to pee often and if he rang his bells a few time and waited and no one took him out I'm sure he'd just pee on the floor. But if he's in his crate, he just sleeps and it's fine. For whoever asked - it's cruel to leave a dog in the crate longer than they can hold it and it's cruel to crate them and not exercise them. Otherwise, don't feel guilty. In Loki's case he can go up to 5 hours, but I like to stick with less than 4. Longer than that and he needs a potty break. He doesn't spend every day in his crate and he goes to doggy daycare once a week and gets walks, so he gets enough exercise. When he was a puppy I couldn't leave him more than 30 mins in the crate or he'd have an accident.
So anyway, how do you know she is not going to the door and waiting for you to let her out when you aren't there? She probably does and then goes to find a spot when no one lets her out.
Oh, and one more tip. Once Loki was crate trained, meaning he wouldn't pee in his crate any more, we upgraded to a much larger crate with a comfy mattress and nice crate cover. When I leave for longer than a trip to the store I give him my big fleece blanket that he and I fight over

Trust me he'd rather be in there than the couch - it's less stressful because he can't see out the windows or anything. I think he likes when we leave occasionally because he can nap in peace!
