Another thought would be a Urinary Tract infection, or even a uterine infection...both of which would smell "fishy" and both of which would require antibiotics, best to take the pup to the vet, if it is just the anal glands, the vet can show your friend how to express them...takes about 2 seconds during the bath, no biggie.
