I am so sorry for your loss. I know it is easier said then done, but try not to beat yourself up. You feel bad enough as it is, but all the "I should haves" aren't going to change things. It wasn't your fault and sometimes we never know why these things happen. Pixie came into your life for a reason. Try to think of all the joy she brought you and know that we are all here for you. RIP little Pixie.
