I never had to make the decision that you are facing - but one of my little dogs was diabetic - and the vet did tell me that there was a good chance that she would become blind later on. But - she went on quickly to say that dogs adjust to blindness very well and can get along very well.
Nevertheless, it is sad -- I'm sorry.
***** If you do decide not to have the surgery - and you have the time and ability to give her a little extra care - she will probably do very well.
