Our yorkie puppy is almost 14 weeks, and we have a similar problem. She always goes in her litter box when she's in the pen (learned that the first day), but when she is outside playing, she seems to forget and go wherever she happens to be when she needs to go.

From what I understand, at that age, it's still hard for them to control their bladder, even if they know where they are supposed to go. We spray both pet deodorizer and vinegar in spots where she has accidents and that seems to at least prevent her from having an accident in the same spot. But I think 4 months is the very earliest one can expect bladder control from a toy breed. And for many, it may be 5 or 6 months.
