I'll just tell you what works for me.
With Kacee, I used bells on the doors, so she could tell me she wanted out. It takes awhile for them to get past the constant ringing.
I took her to the door often, showed her how to ring the bell, and asked her if she wanted to go potty. She caught on quickly. Within a day or two, she was ringing them by herself. Usually,l just to go play, but there comes the time of the eventual payoff when they will ring it to go potty, but of course, they all like to go out to play, too. It's just not a constant ringing anymore.
After she would potty, I got really excited, jumped up and down, and carry on so that she realized she had done good. They just want to please you. So, when they see how very excited you are after they potty everytime, they connect it to something that pleases you.
I, also, always said, "Good girls get cheese." I gave her a little piece of cheese after every single potty. Of course, now she won't come in unless I say it!
I can still remember the day that she "got it" and knew she was doing something she wasn't supposed to be doing. She had peed on the carpet!

I saw her and I went and got the cleaning supplies and kept her right beside me. I lectured her the entire time I was cleanining the spot and she was looking right up into my face listening to me. She had gotten it and I knew it. She was somewhere about 4 months old then and did not have access to the entire house - just part of it. I never used crates. I hate them. But that's just me. The only two accidents she ever had after that was right after she was spayed, when she had two accidents the day after I brought her home, but I just figured that was from the surgery. It never happened again and she is now nearly 18 months old. I hope this might help you some. I know how frustrating it is, but they're well worth it.
