It could be a hemoroid(sp) of sorts. I have seen many tiny pups come into the vet where their rectums have started to fall out. IT is possible to have it pshed back in , but sometimes it requires a few stitches to hold it in. It canbe caused from straining or from being cleaned to vigorously by mom. If she is in pain and not herself than for her sake you should take her in. Sorry to hear she is sick.
