Do you mean the pup is out and still attached with placenta is inside..if so this is not a problem. Simple clamp and cut..the placenta can easily be removed with the after whelping shot..or a simple massage of the uterus...
You can also try to gently pull the placenta..hold it with a dry cloth for a good grip...most slide out..if not clamp and cut it.

Never had a placenta remain if a shot was given or a massage..it is like removing sausage from a casing..slide it out by hand ...
