I'm so sorry to hear that it's possibly the end for your poor dog. I know what you mean about them letting you know when it's time. I just know in my heart that when I finally took my 17 year old doxie, Nando in that it was the right time. I could not bear it if I'd waited till he was in pain or had some type of attack.
