Have you considered tube feeding her? Call some tiny puppies for sale in the paper and see if you can get a hold of a breeder who knows how to tube feed. It's not as hard as it seems and they can show you how (your vet can do this too - don't know why he hasn't suggested it). If her breathing is fine, I think I would definitely try.
