Default newby needs advice

hello, everyone, great forum.
Anna Belle is 5 months old, we have carpet, so at night she is in a crate that is approx 18" wide and 24" long, she typicaly wakes me up with a wimper or a soft short bark to go out, she has had an occassional accident during the night but it has been a week or so since that happened.
during the day she stays in our kitchen in her pen, she has a fold up pen that is approx 2'x6' she pees and poops pretty often in this pen, she is verbally scolded when she does this. we will take her outside several times through out the day, we give a treat and praise when she does potty outside, but sometimes she will do a quick squat and come looking for the treat. we bring her inside and she pees in the pen 15 minutes later, this happens several times through out the day. we really don't want to do the pee pad thing, we prefer the business be done outside.
where are we going wrong???? i have heard some say the crate needs to be smaller, what about the pen, should it be smaller, if so how small??

any other advice will be greatly appreciated.
