Poor baby! I hope the vet can help. A friend of mine also has a yorkie and she had something similar behind her right front shoulder.
Her mommy bought a baby's "onesie" and put it on her so she couldn't scratch and it healed fairly quickly. It hasn't happened again, but she keeps the "onesie" on hand.
