I don't have any personal experience with this, but through research I have found that the bacteria can enter the milk supply so I think I would go ahead with the spay and start bottle feeding the puppies. They're about 2 weeks now and a healthy size so it would only last a couple of weeks before they are eating on their own. Because of her age and the number of large puppies, weaning early will also benefit the Mom.
If Megan is not acting lethargic and eating well, maybe it's not that bad and can wait until a couple of weeks, but I would only let a new vet make this judgment call. Maybe they could continue to give her antibiotic injections for a couple of weeks and test the milk for bacteria and hold off on the spay.
When your vet became mad at you, I wrongly made the assumption that he advised a spay and you were against it so he didn't want to be responsible if Megan died.
