Sorry about your experience. Don't freak out too much about the fact that they have your name address and number. They probably live in another state, or far enough from you. As long as they don't know your financial info or social security number, I think you should be fine. We leave so much info for magazine subscriptions, store catalogues and whatnot that I stopped worrying about that kind of info being out there. The good thing is that you realized what was going on before you got hurt.
