I don't have any great advice but I can say what we do. We do punish Reagan if she goes in the floor. I don't yell or hit or anything I just tell her no then take her and put her in her pen (a pet taxi). It worked great for us. I really started doing this just to get her out of my way while I cleaned it up and to let me calm down just a bit. After a while she was doing really well with the potty training so I started slacking on the pen then she got worse so we started the pen thing again and she got better. She knew if she went on the floor she would be put in her pen (about 5-10 minutes) and she didn't like it, so when we use the pen method she is much better at holding it and letting us know she wants out.
The other thing is you say he rings the bells to go out. If you can and have the time (and don't already do this) I would take him out on a leash when he rings the bells and stay with him for a short amount of time, what you think is adequate enough for him to have gone, then bring him back inside. I wouldn't just let him out on his own because then he is probably just going out to play and not actually going potty. This way he will learn that when he rings the bells its for him to go potty not just go out.
Other than that I know a lot of people use the belly bands, that might be something worth looking into. I have a female so of course my experience is different so you can take my advice with a grain of salt. Hope you find something that works, good luck.
