You said that when you're home you give her free range?? I suggest that even when you are home, you need to section off a specific area for the pup to be in. Having free range at your pup's age is too much right now. My pups are 8 months and 3 1/2 months, and when I'm home they are sectioned off the living room area only.
If sectioning off a certain area will not work for you, try keeping your pup on a leash and tie the leash to you pants. This will ensure the pup is near you at all times and you should be able to catch the accident.
