Default Very Painful Bowel Movements - HELP!

My dog has been having really painful Bowel Movements today; he is almost always silent, but today he's been letting out one ear-piercing yelp after another.

At first I thought he was constipated. Then a vet told us to check his rear for fecal-matted fur, which might be blocking his movements. Sure enough, that was the case; I took care of the matting, and he let out some feces - but it was odd looking: less solid than usual, more green, and sort of curdled looking. 15 minutes or so later, however, he tried to pass some again, but once again the horrible yelps of pain, and nothing came out. I checked his rear, and his anus seemed swollen; I wiped it and encountered some blood. The area seems so sensitive that he even yelped later when he was just trying to pee. Now, he seems very lethargic.

Please help! Does anyone know what is going on or what I should do?
