I've had Japanese Chins with this problem. I was always afraid of anesthesia because all had heart murmurs. So I took a wait and see additude. They never required surgery.
I always get a second opinion but realize the vets in my area are more dedicated to making money then anything else, so I usually make my decisions by my dogs behavoir. If I'm in doubt , I visit a vet in another state.
