I just have a pet door in the wall that leads to a secluded patio for my Yorkies and they all love it. I did build a small ramp on the outside and covered it with that fake grass. This wouldn't be possible with one in a door so I would see how high the drop off is going to be on the outside and make your decision based on your dogs size and bravery.
