They really shouldn't be able to create suction, so maybe his is not very severe and, of course, there is no true substitute for mom's milk. Is it a strong sucking reflex? I think I would weigh him before and after nursing to see if he is getting any milk. Also weigh another puppy to see his change of weight. If the clefted baby's weight is less I would begin supplementation. The milk around the mouth really doesn't prove he's eating enough.
