I don't know about the red thing, but my parents dog used to pee when their friend came over (always same friend) and he worked at a morge. My dad always suspected that it was the smell of the guy. So maybe if they "smelled" like medician or something, something we couldn't pick out but that your furbaby could it could make them pee.
Just a thought.
