I opted to have Cali spayed by a Board certified surgeon because she alos needed a liver and intestinal biopsies. My regular vets were not equipped to fix a liver shunt if they found one. I intereviewed the surgeon who also owns the Animal emergency clinic and was very impressed. They monitored Cali for 48 hours and if I had had her done at my regular vet she would have stayed the night with no one there and she did have some problems during the night that they were able to address. It cost way more but I felt good about the vet. For routine spay and neuters regular vets are fine but I would make sure you have confidence in them. The not being clean would worry me and his age.
