Loki is totally outside trained. He had weekly accidents until he was about a year old, when we put bells up. Bells rock. Now he is about 16 months and he is completely trained - meaning months without accidents. He actually had an accident last weekend and i rushed in a urine sample to the vet because he is SO good that I figured he had a UTI. (His accident was in front of the door - when he has to go he HAS to go and I think one of us didn't hear him.) We started off with several UTI's when he was a puppy, and he also has nighttime incontinence issues he is on drugs for AND he drinks about twice as much water as he should - so he is a pee machine. It is absolutely possible to get them completely trained, it's just a lot of hard work. Good luck.
