Small dogs can go into their first heat cycle as early as four months of age, although six months is more common. So your pup could very well be going into heat.

Call your vet and discuss the timing of her spay surgery. There is a somewhat higher incidence of surgical complications when dogs are spayed while in heat, due to the swelling in the surgical area. Your vet should be able to discuss the pros and cons with you.
