I’m not a lawyer and can’t give legal advise, but if I were in your position I’d send the vet a very strongly worded letter detailing your complaints against her and telling her you will be filing against her in small claims court if she doesn’t provide you with an itemized bill and work with your insurance company. If she still refused, I’d file a claim in small claims court. The maximum claim you can file there is 10,000 — but you probably owe her something for her work, and you won’t need to pay an attorney, so it would be more cost effective in the long run.

I probably would also contact my credit card company and dispute the charges. At the very least, they would likely agree that a charge that high warrants an itemized bill, which you are justified in asking.
