Wow. If this poor dog really *cannot* poop, then yes - eventually it will progress to very serious trouble bc the feces will back up further and further into the intestine - and I mean it can and will back WAY up...as far as it needs to go. And when it sits up there for an extended time, it becomes very dry and very hard -- and eventually an enema cannot reach it. It will become an impaction.

If this dog cannot poop, there is most definitely something wrong here. Poor, poor baby .

My guess is that the Vet is probably suggesting way more be done here (like a scope..?) and perhaps the owner doesn't want to pursue it....?
