I discovered at about 1 year that my Jillie had a mild luxating patella. I was pretty devastated and pulled her out of agility training.

I was told that at her weight -- she's about 5 1/2 pounds now -- that it might not be a problem. I gradually allowed her to do more running and playing.

I'm happy to say she turns 9 next week, and other than a couple times when the leg momentarily locked in place, it has never been a problem. It always popped back in and she returned to running. No apparent pain.

Personally, I wouldn't do surgery until it becomes a problem, bu a vet might advise you otherwise.
