Take the pad/blanket out of the crate. When she goes to potty there, give a firm no and redirect her to outside where she should be pottying outside. If it were me, I'd remove the potty pads in totality so she's not getting confused. Limit her access to only the areas you want to protect any carpets etc.

Get a different dog bed to (change style and color) that may help with the smell that you may not be getting out when washing the dog bed from the urine smell.
