When this happened to Ella (when she was 4.5 months), she squealed a couple of times trying to pee on her pad. I think that scared her so she held her pee for a long time, and just relieved herself everywhere else but on her pad. She had a bladder infection.

I also ended up confining her in her pen area (it's a 6'x4' area) until she was consistently peeing on the pad again.. it just took a day.

Maybe there's something health related with you pup?
