At 4 months, you need to keep her in a restricted area with the pee pad. If you give her free access to your home before she has learned to potty only on the pads (or outdoors), then you will inevitably get accidents.

Only let her roam freely after you have seen her potty, whether on the pad or outdoors. She needs a strict potty schedule too. If she still has accidents, it means that she doesn't get relieved enough or you've given her too much access. You'll want to keep up with the strict schedule and limited access for a good few months until you see no accidents.
