Depends on the dog. Some Yorkies do need one every year.

Do you do maintenance in between? Brushing teeth daily etc?

It does make a huge difference. I got lazy with Jackson and stopped brushing for a few months and now he needs a dental again. He had one around.... 2 years old, 3 maybe? And that was for a different surgery (dental related) so they cleaned them while under. Second dental was at 5 years old. Now he's 8 and going to need one this year. I swear every time I'm going to keep up with them after the cleaning but this time I really mean it because as he gets older I'd prefer not to put him under anesthesia unless absolutely necessary.

Other products I like are HealthyMouth (VOHC approved -- they have a mouth rinse you add to water, a gel, and a spray). I use CET vanilla mint toothpaste. And some dental chews that probably don't do much (Dentastix every once in a while, etc).
