Dogs in most parts of the US need some kind of heartworm treatment at the very minimum. There are alternatives to Trifexis. We use Revolution, which is drops that you put on the back of the neck once a month. Here is an article by a vet clinic: Animal Medical Clinic - Veterinarian In Fremont, NE USA :: Heartworm Disease . They recommend Heartguard or Revolution.
