Well, I am not a vet, but here goes:

1. I would take him to another vet for a second opinion.
2. Once there, I would request that he be screened for parasites and worms.
3. I would also request that his anal sacs be checked to see if they need draining.
4. If the problem is neither of these, I would look in allergies as the source.
5. Finally, you can use Apple Bitters and spray them on his "hot spots" so that he doesn't scratch, or another product is Dr. Harvey's Healing Cream.
