To me, this sounds like a localized pyoderma infection (aka staph intermedius, aka a "hot spot"). This infection could easily go systemic, btw.

Does he have allergies or something? Does he chew/lick his paws?

For localized pyoderma, the treatment is usually Muciprocin ointment, which is prescription. If systemic, then it's Simplicef x 30 days. You could try putting antibiotic ointment on it 2x per day and see if that helps since it's still early.
