Believe me , IF this dog is suffering from Kidney or Liver failure he most certainly is in pain as both conditions are very painful. Also there is no way for you to know if this dog is in pain as animals don't show pain until it becomes unbearable & even then often they will still try to hide their pain, it's the nature of the beasts. At his age he most likely IS in organ failure & if she's choosing to not treat it then she needs to take him to the vet & humanely euthanize him. I was a vet tech for many yrs & I can tell you just by what you have posted , this dog NEEDS to see a vet ASAP, either for treatment or euthanasia.
