I have never tried a pad holder or frame, but it sounds like a great idea! I usually put two pads together so that they overlap. This often does the trick.
I did notice the same tendency you described in a senior Yorkie I had. Her eyesight was not as good as it had been and I think that affected her hitting the pee pad.
