My first thought was IBS/IBD as well. Some vets will treat 'as if' with a trial of meds, and not all dogs get a colonoscopy but still get treated. Metronidazole is one common treatment for IBS/IBD, and it sounds like that's working for your dog. I've heard of dogs staying on low dose metro long term to treat the disease.

My dog first went on a chicken and sweet potato diet after he was diagnosed but it did not work well at all for him. He can't handle chicken. We decided to try a novel protein that he had not had before and tried a few before we found that lamb worked well for him. Its not typically a diet that works, but my little guy is thriving on Primal frozen lamb patties. More than a bite of anything else is likely to cause his tummy to flare up, but as long as we stick to the lamb he's fine. When he does occasionally flare we use budesonide. Metronidazole didn't work for him.
