I'm not experienced with relapse in peeing in the crate. Scottie had poop accidents, but only when he had an upset stomach.

I'm not familiar enough with vaccines, so I'm not sure if this is a side-affect from the vaccine she had last week. Maybe others can comment on possible health related issues. Or maybe ask the vet if there's a possible urinary-related issue?

I was surprised to hear that you withhold water from her during the day though. I have always read suggestions to withhold water after a certain time at night before bed when potty training, but that they should always have access to water during the day. During crate training, I hung a dish of water inside Scottie's crate and only took it away after during night time.

Alternatively, if it's a matter of her not being able to hold it in or not being a health-related issue, maybe you can consider getting an x-pen and laying out pee pads during the day?
