To me, this looks *exactly* like pyoderma, which is a staph intermedius infection. This is seen recurrently in dogs with allergies, Cushings, and/or thyroid issues. It's treated w/ Simplicef x 30 days if diffuse, or perhaps Muciprocin if local only.
