I think a lot of trouble arises as some do not read the rules and conditions of coverage carefully enough. Choosing an insurer is a complex and time consuming process and it is hard to compare apples-and-apples coverage with different companies. For example, some cover hereditary issues, some do not. There are so many pitfalls for the unwary, as most of us are when we first search, but many here ask for advice and experiences and get good insurance as a result.
The important thing is to insure a dog young before it shows any health pre-existings, then the results are usually ok.
