I would tell the vet's office your preference. On the one hand, it's just a piece of paper at your vet's office and doesn't really make a difference in health care. But, I understand it being bothersome.
The first time I took Max to our current vet's office, the receptionist asked me to identify his breed. I said Yorkshire Terrier and she said that his ears were "enormous" so he didn't look like a Yorkie. Being a proud momma, my feelings were hurt.
