Lots of good advice above to relax and let your puppy be himself.  Max preferred to sleep in his xpen his first year.  Now he cuddles with me in bed.  
 
Puppies are often high energy and don't want to feel trapped.  Be easy going and give your pup the sense that he has his freedom.  I trained my boys to come by making sure that it wasn't always for things they didn't really want.  I called them to come, gave them a reward and/or praise and then let them go on their way.   
Regarding names and name recognition -- if he continues to not respond to his name, I would consider a name change.  Our breeder told us that she sometimes changed names for pups who stayed with her longer than 12 weeks and who didn't respond well to their original names.  We brought home Teddy from her when he was 8 months.  He was called Tank, and had another name before that.  He responded immediately to Teddy and just loved it!  Just a thought.   
Be cheerful and silly with your puppy, make the training fun, and the affection will come.
