That looks pretty wet and raw! Does he lick himself a lot? Do you take him on walks where he may be coming into contact with an allergen? Does it seem to itch? I would ask the vet about using betagen to dry it up. If it continues I would insist on a skin scrape and or swab to test for skin infection that may require antibiotics.
