Try going backt to the beginning of what worked previously. If he still gives you a lot of issues maybe it is medical.
He really only has problems when I'm not home. My dad is here a majority of the day but yesterday both parents were home and he snuck under the table and pooped! He used to do that and get a stern no and 5 mins outside so he quickly learned to poop while outside after those days! (That was right after he became officially potty trained)
