Well you don't push anal glands back in so I'm wondering if the pup has a prolapsed rectum ? Can be caused by straining to poop , also can be caused by bladder stones/infection. I think I would find another vet ASAP. Yes a 3 mo old puppy can have impacted anal glands & again this is caused by allergies and/or irritation of some kind, such as contact. How often do you bath him ? Do you rinse well in that area to be sure there is no soap left there ? If there is an irritation there then it's most likely very uncomfortable for the stool to pass & is why the yelping & running in circles, trying to run away from the pain. Did this pup recently get a hair cut ? if so it may be irritated from that.
