A few things...what happened w/ Atopica...usually that works wonderfully. Have you tried Apoquel yet? What happened also when you used steroid? To me, this looks like pyoderma, which is an overgrowth of staph intermedius on the skin...it is diagnosed via a very simple skin smear...then treated w/ Simplicef x 30 days. Has that ever been done.
The other thing you should look at since these are recurrent is Cushings Disease - read this overview and see if it fits: Cushings (Hyperadrenocorticism)

Agree with all above....my brother had a beagle that he was flying a skin specialist vet from Dallas to Houston to provide care for his beagle....poor Sparky was sooooooooo miserable and his skin was in deplorable condition, itching, oozing, he was a hot mess. It cost him a fortune, but he adored Sparky, and that dermatologist finally got poor Sparky where he at least was able to tolerate his severe allergies to grass/fleas/chiggers/various tree and flower pollens......Sparky's condition was a direct result of poor breeding, which we were completely ignorant about.
I would highly recommend you get with a specialized dermatologist vet that deals strictly with skin conditions/allergies.....these poor unfortunate dogs can be absolutely miserable with these skin conditions, as we wring our hands and even cry over watching them in their misery.....hang with it....I would skip over alny vet that is NOT a specialist in skin conditions.....I think thay will save you wasted money while you "try this" now "try that"....
