Welcome to YT and thank you for rescuing this sweet little girl and getting her healthy. It could have been the smells of the pet shop, smells of other dogs and animals, she could have feared she was going back to the mill.
At one time I had a pack of 3, all purchased as 8 week old puppies, the first time I took them into a Pet Smart shop ( they were 2, 5. & 8 yo) you would have thought they had just walked into the vets office,they were all shaking and trying to climb out of the shopping cart, so out of the store I went. I waited over a year to try it again, same reaction, they were terrified, I think it was all the diff,. smells, they were all very dog friendly so I know it was not the smell of other dogs. Try taking her to a Lowes or Home Depot, any store that allows dogs, or you can walk her through town, see what her reaction is.
